Retooling D-SNPs: Demonstration Options for Leveraging D-SNPs for Better Chronic Care Management, Behavioral Health Management, and Aging in Place
Members of the Association for Community Affiliated Plans (ACAP) have extensive experience serving dually eligible beneficiaries through Dual-Eligible Special Needs Plans (D-SNPs), Medicaid Managed Care plans, and Managed Long-Term Care Services and Supports (MLTSS) plans. They routinely direct more resources to medical care than other plans serving the same populations. ACAP members are uniquely aware of the complex chronic care needs, behavioral health needs, frailty conditions, aging in place limitations, and long-term care support needs of dually eligible individuals. Thus, ACAP’s member plans are uniquely positioned to identify innovative solutions and opportunities for improvement.
This paper is the result of a months-long effort by ACAP and its member D-SNPs to identify ways to improve the Medicare and Medicaid programs for dually eligible beneficiaries. These ideas, all of which would be voluntary plan demonstrations, are intended to extend the D-SNP care management model to other Medicare beneficiaries, test the role of D-SNPs in providing housing supports to facilitate Medicare beneficiaries’ aging in place, and improve Medicare and Medicaid integration for dually eligible beneficiaries.
