The Association for Community Affiliated Plans advocates for publicly-supported insurance programs. ACAP’s member Safety Net Health Plans serve people who are enrolled in Medicaid, and serve more than 30 million people nationwide.

In 2024, Safety Net Health Plans reported performing at a significantly higher level than other Medicaid managed care plans on 42 HEDIS indicators of quality of clinical care.
Safety Net Health Plans performed at a significantly higher level on indicators of diabetes care, cardiovascular care, cancer screening, and immunizations for children and adolescents.

| AlohaCare has contributed more than $21 million in scholarships, grants, sponsorships, and charitable donations to people and organizations across Hawai’i.
Among their investments: Mana Mama, a mobile maternity care clinic capable of serving up to 30 patients a week. Today, four mobile clinics serve Oahu, Hawai’i, and Maui.

| Alliance Health (N.C.) committed $25 million to expand behavioral health, food and housing support; the plan Alliance Health partnered with the Steve Smith Family Foundation and Mecklenburg County to open a new behavioral health urgent care center in Charlotte.

| VNS Health (N.Y.) invested $69 million in charitable care and community impact programs in 2024. The plan’s programs supported in-home care for people like Violet, 87, who worked with a physical therapist to regain her mobility and help restore her independence. | |
| L.A. Care (Calif.) launched a $255 million initiative to address the physician shortage in Los Angeles County. Between full medical scholarships, recruitment and retention grants, the Elevating the Safety Net program has brought more than 200 physicians to Los Angeles County since its inception in 2018. |

| The CareSource (Ohio) Foundation has invested more than $32 million in eliminating poverty, providing needed services, and fostering healthy communities.
CareSource’s investments include a $1.2 million partnership with the Children’s Hunger Alliance to address food insecurity. |

| Central California Alliance for Health invested $93 million in housing, workforce and education in 2025. Among their investments was $30 million for housing, creating 824 permanent supportive units and 210 temporary beds. | |
| Fallon Health (Mass.) invested $2.5 million to address food insecurity, support older adults and more through its Community Impact Grants program. | |
| Over the past 5 years, Community Health Plan of Washington has invested more than $4 million in lowering barriers to care through language support, transportation, and patient education.

| In 2025, the AmeriHealth Caritas Foundation awarded grantees $2 million in an effort to help prevent adverse childhood events. The Foundation supported, among many others, Children Trust Michigan to help fight child abuse and neglect in the state, as well as Community Renewal International‘s youth programs in Shreveport and Bossier City, La. |
