Nest Health is a value-based primary care provider designed for families on Medicaid. We deliver comprehensive medical, behavioral, and social care in the home and virtually for all family members, from pregnancy through adulthood. Our interdisciplinary care teams include nurse practitioners, behavioral health clinicians, upskilled medical assistants, and nurses who address both clinical and social drivers of health. By bringing care directly to families, we remove common barriers such as transportation, childcare needs, and work constraints that often prevent preventive and follow-up care.

Since launching in 2022, Nest Health has partnered with managed care organizations including AmeriHealth Caritas Louisiana, Aetna Better Health of Louisiana, and Blue Cross Blue Shield Arizona Health Choice to improve engagement and outcomes among families on Medicaid. Early results show a 60% reduction in ED utilization, high rates of postpartum follow-up, and vaccination rates 2x the Louisiana state baseline. Our model emphasizes coordinated family care planning, data-driven risk stratification, and culturally responsive engagement to improve health equity and deliver measurable quality improvement for families on Medicaid.

About Nest Health’s SDOH Services: 

Nest Health integrates social care directly into our clinical model. Every family completes a social needs assessment that informs a shared care plan addressing issues such as food insecurity, housing instability, transportation, and utility access. Being in the home allows our care teams to see what clinic-based care often misses, including unmet needs, environmental challenges, and family dynamics that directly affect health. Our family advocates and mental health specialists work alongside clinicians to connect families with local and state resources, support benefit navigation, and address those needs in real time. By identifying and resolving these social drivers of health alongside medical and behavioral care, Nest helps stabilize families and improve engagement, adherence, and long-term outcomes.
