Nadia Care is a maternal health company dedicated to assisting mothers across the entire prenatal and postpartum spectrum. Using a hybrid care model that integrates community-centered, in-home care and virtual supports, our Maternity Navigators and multidisciplinary care teams facilitate comprehensive care coordination, enrollment assistance, breastfeeding guidance, nutritional counseling, and prenatal and new parent education.

Our integrated approach has demonstrated measurable clinical improvements, including a meaningful reduction in NICU use and costs, low birth weight rates, postpartum depression, preterm birth rates, and emergency room visits. About Nadia Care’s SDOH Services: 

Nadia Care supports members with a range of SDOH needs through a community-centered model that relies on local Maternity Navigators, doulas and other providers to screen for and provide wraparound care. Delivering virtual, in-home, and community visits, our navigators build trust with members, connect families to vital resources like transportation, housing assistance, food programs (WIC/SNAP), and baby supplies. Along with RNs, lactation consultants, dietitians our team provides empathic, culturally-informed services that ultimately improve member experience, reduce ED visits, preterm births, low birth weight rates, and NICU stays and costs.
