Helios Connect is a community hub with a Social Care Network (SCN) connecting community organizations offering health related social needs (HRSNs) and Managed Care Organizations (MCOs) /health plans and Providers. Our SCN includes organizations offering re-entry services, housing and supports, medical respite, transportation, employment and workforce development, food and nutrition services, behavioral health and other wrap-around services that are desired by the client to be contracted within the network.
Our services include:
1) a closed loop referral and member engagement technology solution between MCOs/health plans, Providers and community organizations;
2) A concierge team that builds relationships and trust with community organizations and provides technical support for community organizations to ensure there is thorough and accurate status updates and manages access and availability of services;
3) A curated network of community organizations offering HRSNs;
4) administrative and back-office support for community organizations and Community Health Workers to support in credentialing, claims submission, eligibility, encounter submission, reporting, and compliance.
Our goal is to enhance current care models and foster improved communication between MCOs/payors, providers and community organizations.

Helios Connect is a community hub with a concierge team, and a Social Care Network of community organizations offering SDOH services by offering a closed loop referral solution and with Managed Care Organizations (MCOs) /payors and hospitals. We can support community organizations and MCOs/payors who are participating in a 1115 waiver. Additionally, Helios Connect offers technical support and administrative services to include Health Workers to support in credentialing, claims submission, eligibility, encounter submission, reporting, and compliance.
