Addressing Social Determinants of Health Through Dual-Eligible Special Needs Plans

ACAP will host a webinar to discuss the report on Monday, November 2 at 4pm ET to discuss our new report  on how Dual Eligible Special Needs Plans (D-SNPs) can address the social determinants of health (SDOH).
D-SNP beneficiaries often have extensive SDOH needs which can significantly impact health outcomes. ACAP worked with the Center for Health Care Strategies to assess how our member D-SNP plans currently address those needs and the extent to which ACAP-member D-SNPs utilize Special Supplemental Benefits for the Chronically Ill The report was funded by a grant from Arnold Ventures.
The report also features policy suggestions to better prepare D-SNPs to address SDOH needs in a meaningful way. You can read the full report here.
