Value-Based Payment (VBP):

HMA, Wakely, Edrington Health Consulting, and Leavitt Partners are leaders in developing and implementing VBP solutions in Medicaid, Medicare, and ACA for community-based plans. We have developed programs targeting PCP, Specialty, Maternity, BH, kidney care and HCBS benefits. Our provider contracting support includes training, facilitation and education for payors and providers, financial modeling, and performance monitoring of VBP programs. We utilize proprietary tools like WAHOO to inform risk score optimization, and provider network optimization strategies.

HMA’s SDOH Services:

HMA offers expert guidance and resources to design, development, and implement SDoH infrastructure, programs, and initiatives with a focus on equity and improving member health outcomes. Our knowledgeable consultants bring “boots on the ground” experience to help plans with SDoH strategy development, operationalizing SDoH plans and programs, and engaging with/investing in community organizations as partners to connect members to the critical non-medical services they need to get and be their healthiest.

HMA’s Retention Services:

Health Management Associates, including Leavitt Partners, is well qualified to assist ACAP plans in executing strategies for the resumption of Medicaid eligibility renewals. HMA’s experts use on-the- ground experience working in state and federal Medicaid agencies and with health plans to identify workable solutions to member outreach, policy, technology, and workforce challenges. HMA can help plans prepare for these major changes with a primary focus of minimizing the disruption of care for Medicaid eligible members.
