We provide scheduled telehealth visits for preventative and specialty services as well as real time transfers to providers for an immediate appointment. During telehealth visits providers conduct detailed assessments to address quality measures along with other clinical needs. Once a visit is complete, we provide care coordination and referrals, as needed, as well as remote patient monitoring services to ensure the patient is getting the care they need when they need it.  Finally, we collect and analyze real time health plan data providing clear insights into patient care. Our dashboards and live client console make it easy for you and your team to track performance and outcomes.

Our staff knows that medical care goes far beyond a provider visit. Our staff seek to identify social needs and connect patients to local resources. On average our typical patient is connected to 4 social services while they are in our care. Identifying and connecting patients to social supports allows patients to live a healthy, happy life.
