As a Management Services Organization (MSO), Certintell partners with health centers, and payers, and delivers comprehensive primary care-led services to patients via telehealth. These health centers and payer patients receive Remote Patient Monitoring services and hardware, including a white-labeled telehealth portal.

About Certintell’s SDOH Services:

Certintell supports SDOH via our care management, care coordination, and remote patient monitoring services. We provide culturally component clinical staff that reaches out to patients where they are via telehealth and Remote Patient Monitoring with a focus on high-risk chronic patients. We also support gaps in care by helping patients navigate available community resources through findhelp.org. We have shown our model improves outcomes and lowers costs for low-income chronic patients on medicare and Medicaid.
