Social Worker (Homeless Response Team)

Position Summary:

CalOptima is a county organized health system that administers publicly funded health care coverage for low-income children, adults, seniors and people with disabilities in Orange County, California. CalOptima’s mission is to provide members with access to quality health care services delivered in a cost-effective and compassionate manner. This position will serve as a lead and a resource on the Homeless Response Team. The incumbent will assess members at risk for homelessness for various programs available. This position provides shared responsibilities for direct communication with CalOptima’s contracted Federally Qualified Health Centers (FQHC) who provide onsite clinical assessment for members impacted by homelessness. Accepts referrals for case management and makes recommendations for appropriate level of care and measures needed to successfully manage care. This position will require attendance at community meetings.

Position Requirements:
 Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, sociology or related field required. Master’s Degree in Social Work or related field desirable.
 2+ years experience working with persons impacted by or at risk for homelessness required.
 Experience working with individuals impacted by behavioral health issues desired.
 Medi-Cal, HMO, and health services experience preferred.
 Valid California driver’s license and vehicle, or other approved means of transportation, and an acceptable driving record will be required for work away from the primary office 50% of the time or more.
 Bilingual in English and in one of CalOptima’s defined threshold language is preferred.

CalOptima’s Social Worker must work to:
 Establish and maintain effective working relationships and communicate with all levels of staff, providers, agencies, and the public.
 Assist in the formulation of program specific processes and workflows
 Develop a member centric plan, for members through multi-disciplinary collaboration which identifies options and goals.
 Advocate on behalf of the individual to assure quality of care and attainment of appropriate goals.
 Effectively utilize computer and appropriate software.
 Prepare clear, concise written and oral reports and materials, including effective charting.
 Ability to prioritize competing deliverables and meet deadlines.

CalOptima’s Social Worker must have the ability to:
 Principles and practices of health care service delivery and managed care, Medi-Cal, CalOptima eligibility, and benefits strongly preferred.
 Principles and techniques to serve persons impacted by homelessness in diverse groups.
 Social determinants of health, trauma informed care, and motivational interviewing.
 Community resources that support individuals impacted by homelessness.
 Discharge planning knowledge desirable.
