Health Navigator – Transgender and Non-Binary

The Transgender and Non-Binary Health Navigator will work with Transgender and Non-Binary (TGNB) Amida Care members to ensure needs related to gender affirming care and HIV prevention are being met on a timely, consistent basis. The TGNB Health Navigator will act as the first point of contact for many GIST members, addressing day-to-day issues and providing navigation services for gender affirming surgeries, electrolysis, PrEP and PEP, and will support the GIST team in programmatic development and expansion.

ESSENTIAL FUNCTIONS
Essential functions are job duties and responsibilities that must be performed to accomplish the purpose/ goals of the job.

• Outreach Transgender and Non-Binary (TGNB) Amida Care members to coordinate referrals, explain benefits, and inform members of new or expiring authorizations for gender affirming medical procedures

• Refer and connect members to appropriate services/service providers

• Assist with verifying which enrolled members require transgender health navigation to best coordinate their care

• Communicate with partnering providers for the coordination of care for Amida Care members

• Assist with navigating members through the electrolysis referral process

• Engage HIV- members about HIV Prevention tools.

OTHER RESPONSIBILITIES
Include other significant responsibilities needed to accomplish job purpose/goals that are performed on a daily or infrequent basis.

• Support the GIST team and Program Manager in programmatic development

• Keep detailed records of member communication and coordination

• Perform other duties as assigned.

MINIMUM JOB REQUIREMENTS

• Bachelor’s degree in Human Services or significant relevant experience in HIV medical case management/care coordination
• Experience working with and providing services to members of the transgender community

• Minimum of one year experience working with those living with HIV in a social service or medical setting

• Demonstrated knowledge of HIV case management and LGBTQ+ cultural competency

• Demonstrated judgment of safety, boundaries and confidentiality issues

• Demonstrate understanding and sensitivity to multi-cultural values, beliefs, and attitudes of both internal and external contacts

• Demonstrate appropriate behaviors in accordance with the organization’s vision, mission, and values
